What Medicare Advantage Special Needs Plans Cover

A special needs plan (SNP) is a Medicare Advantage plan that is designed to provide specific care while limiting enrollment to special needs individuals. A special needs individual under Medicare plans include an institutionalized person — long-term care, or another facility — dual-eligible (Medicare + Medicaid), and people with a severe or disabling chronic condition listed in the SNP plan

An SNP includes may be: Chronic Condition SNP (C-SNP), Dual Eligible SNP (D-SNP), Institutional SNP (I-SNP) depending on your qualifying condition. Eligibility is based on condition and may not be restricted to age requirements like traditional Medicare.
